Ankle Foot Orthosis Gauntlet: Clinical Benefits and Targeted Foot Pathologies

An ankle foot orthosis (AFO) gauntlet is a circumferential, semi-rigid brace that merges with a custom-molded thermoplastic inner shell or is constructed from 3D printed TPU (Thermoplastic Polyurethane). An AFO gauntlet envelopes the foot, ankle and distal tibia, delivering “total-contact” support that is difficult to achieve with shorter, open-design braces. This unique construction translates into measurable clinical benefits for patients within a well-defined group of foot and ankle pathologies.

Tillges Orthotics & Prosthetics has designed and developed a custom 3D printed ankle gauntlet brace called the Comfort 3D. This new design allows flexible soft plastic material (TPU) that cartels the boney structure of the foot and ankle to maximize comfort and stability in the ankle and foot. We have taken traditional bracing and have blended modern technology and materials to form this new line of bracing that not only improves comfort and compliance, but also has improved stability and outcomes for our patients.

The Tillges SMO 3D brace is a custom-fabricated SMO that stabilizes the ankle to provide medial-lateral ankle support with limited plantar- and dorsi-flexion. Provides stabilization to the ankle, subtalar and mid-tarsal joints. The SMO 3D is lower profile than the standard Gauntlet AFO, which extends just promixal to the malleoli.

The Tillges Gauntlet 3D Comfort brace is a custom-fabricated AFO that stabilizes the ankle to provide medial-lateral ankle support with limited plantar- and dorsi-flexion. Provides stabilization of the ankle, subtalar and mid-tarsal joints. The Gauntlet 3D Comfort is taller than the SMO, which extends an additional 3" proximally.

Key Biomechanical Benefits of an AFO Gauntlet:

Lower Profile & Volume Tolerance
Compared with a standard solid-ankle AFO, the circumferential and low-profile design and thinner plastic or comparable material shell makes it easier to fit into shoes.  The lace or Velcro closure can be loosened to accommodate daily swelling, which is an advantage over rigid polypropylene AFOs. 

Triplanar Stability
The gauntlet crosses the ankle joint and extends from the metatarsal heads to above the malleoli. The long lever arm controls sagittal, frontal and transverse-plane motion, reducing unwanted talar shift, hind-foot eversion and mid-foot collapse.

Load-Sharing & Pain Reduction
The semi-rigid shell absorbs dorsiflexion torque in late stance, unloading attenuated ligaments, arthritic joints or a failing posterior tibial tendon. This reduces tibiotalar motion and subtalar motion, which directly correlates with lower end-range pain in arthritic ankles.

Accommodation of Fixed Deformity
Because the device is casted while the foot is held in the best attainable neutral position, flexible components can be corrected, and rigid components can be “accommodated” without high focal pressure. The custom fit gauntlet further dissipates shear forces, with its total contact design, making the gauntlet ideal for patients with fragile skin.
